Here is a simple case.

87 year old lady scheduled for a carpal tunnel release under mac...or really 3 minute general because the hand guy is SOOOO fast.

On day of surgery the preop nurse comes to me and tells me that this patient's admission vitals include a heart rate of 160 beats per minute.

She's obviously stable....she walked into the hospital with her daughter and appears to be a healthy elderly lady.

Meds: diovan and norvasc for HTN.....nothing else going on.
