Recently I've been getting some thoracic cases, which is fine with me as I think these are nice, challenging student cases (a-line, epidural, DLT, lots of physiological stuff going on).

So, here's my questions, for a thoracotomy , you put in your epidural preop, give it a test dose. Then do you dose it during the case? I'm being taught that you NEVER, EVER dose it during the case in case you run into major bleeding (i.e they nick the PA ).

What are you guys doing? Any good research on the subject?
