OK, Ive just got to say this, and Im sure that its been beat into the ground, but this Student Doctor Network site blows me away. I recently started reading through some of the threads there more for educational purposes. As you are already aware, these people not only put CRNA's right above cockroaches, but also general surgeons, ENT's and so on. Some of these people are completely delusional. I am neither a CRNA or a Medical student, so I feel that I am pretty balanced on my opinion. Some of the best and most intelligent medical professionals that I have ever worked with are CRNA's and in 95% of the cases I would rather have a CRNA working with me as opposed to an MDA. I currently work with 4 cardiovascular surgeons who would eat someone alive if a foul word was even mentioned about their CRNA's. The MDA's on the otherhand come to my unit and gripe and complain about one another and how they dont get any respect from other physicians. Sorry to rant. Im just sitting here in amazement laughing about the things I have read.

Joint Commission Cracks Down on Bullying
The abusive, manipulative and just plain nasty treatment that many healthcare professionals get from their peers on a daily basis has got to stop, says the Joint Commission in a new Sentinel Event Alert, the first to deal with the hot-button issue of lateral violence in the workplace.
"The inherent stresses of dealing with high stakes, high-emotion situations can contribute to occasional intimidating or disruptive behavior, particularly in the presence of factors such as fatigue," warns the alert. "Individual care providers who exhibit characteristics such as self-centeredness, immaturity or defensiveness can be more prone to unprofessional behavior."
This disruptive behavior, which can range from verbal and physical aggression to passive hostilities like refusing to cooperate with colleagues, can heighten the risk of medical errors and poor outcomes, reduce patient satisfaction, increase costs and drive quality professionals out of the business, says the commission.
"It is important for organizations to take a stand by clearly identifying such behaviors and refusing to tolerate them," says Joint Commission President Mark R. Chassin, MD, MPP, MPH, in a statement.
To that end, the commission has established 11 steps for accredited hospitals, surgery centers and other healthcare organizations to take by Jan. 1, 2009, including the following:
educate and train all team members, including clinicians and staff, on appropriate workplace behavior;
hold team members accountable for adhering to and enforcing the facility's code of conduct;
develop and implement procedures to address lateral violence, including a zero-tolerance policy, a reporting and surveillance system and non-retaliation clauses to protect those who witness and report disruptive behaviors; and
address problematic behaviors with non-confrontational intervention strategies focused on rehabilitating the abuser and preserving patient safety

I look at the Joint Commission (AAAHC, etc.) much like Realtors: These are people, who from nothing, created an illusion of a need for their "services" and then built a thriving multi-billion business filling that "need" (which, as I said, they themselves created). It simply makes no sense. Do I really need somone to walk me into a room and say, "Now, this is the kitchen......"? Quite honestly, the stove gave it away as soon as I walked into the room. The Joint Commission operational philosophy appears to be based on the Realtor's Handbook.
Now, that is not to say that the Joint Commission hasn't done some good over time, but the cost : benefit ratio is tremendously high. I'm thinking that money could be better spent.
As far as Realtors as a group or "profession" (a word which is way too far over-used), I'm pretty certain they have done nothing but harm. I have more respect for Amway sales people. At least when you finish with them, you have some soap to show for your money.......

There is a thread on SDN regarding this topic and the view over there is much different than here. The docs there are actually quite supportive of this particular directive by JCAHO. I would have expected the opposite to be true, that folks here would support it and the SDN members would call it B.S. Very interesting.

I find that odd since the bullying that most often happens is MD -> RN.

That really doesn't surprise me at all. It is probably related to the nature of the population on SDN: Medical Students, Interns and Residents, who are frequently bullied within their vertical pecking order by their upper class men and staff physicians. Most CRNAs do not bully SRNAs.
I'm certain those poor little fellows on SDN get their feelings of entitlement and superiority hurt pretty often.......

That really doesn't surprise me at all. It is probably related to the nature of the population on SDN: Medical Students, Interns and Residents, who are frequently bullied within their vertical pecking order by their upper class men and staff physicians. Most CRNAs do not bully SRNAs.
I'm certain those poor little fellows on SDN get their feelings of entitlement and superiority hurt pretty often.......I briefly considered that as a reasonable explanation when I first viewed the thread. However, once I sifted through the pithy, sarcastic comments I found the the ones who supported it were a seasoned AA (jwk himself) and a ~10 year private practice MD. It's not an avalanche of support by any means but still more than I thought I would see over there and even more than what's been posted here so far. *shrug*

Why is it that every thing that JC ever 'cracks down on' just results in another piece of time-consuming paperwork for the RN to do thereby shortening even further the time that they have in their day to do any REAL PATIENT CARE?

agreed.. but they DO make the hallways free of equipment. i swear management has C-arms in their garages and stretchers in their living rooms when JC's in town.

I have actually seen an outpatient surgery center administrator with boxes of incomplete medical records in the trunk of his car when the Joint was at his place. So many things change when they are around and then everything goes back to business as usual. It kind of makes a mockery of the whole process......
